Create your Simple Practice account.

1. Click on this link to complete the form: https://solpsychiatry.clientsecure.me/contact-widget

2. Select Aaron Cummins, PMHNP from the list of providers.

3. Upload a photo of your insurance card (this is required so we can verify your benefits and successfully submit claims on your behalf.)

4. Once in our system, we’ll send you a client portal link to access the Sol patient paperwork. This will make you an official Sol client (and required for any refills).

5. Once the paperwork is complete, you’ll have access to schedule an appointment with Aaron through your client portal.
