IMPORTANT NOTE BEFORE FILLING OUT OUR NEW PATIENT REQUEST

Limited New Patient Availability in our Wimberley office. We are currently accepting new patients on a case-by-case basis, based on provider availability and clinical fit. Our Wimberley provider has a 6 to 8 week waitlist for new clients.

Because our Wimberley provider is a nurse practitioner, there are certain prescribing limitations that may affect whether we can continue all of your current medications.

We are unable to prescribe opioid medications. We may not be able to manage combinations of opioid medications with other controlled substances, such as Xanax or Adderall. Our prescribing practices also follow FDA maximum dosing recommendations.

If your current treatment includes these medications or combinations, we recommend establishing care with an MD through LifeStance Health or Wimberley Medical Clinic.

We appreciate your understanding and are happy to answer any questions about whether our practice is a good fit for your needs.

We accept the following insurance plans:

  • Aetna (most plans)

  • Blue Cross Blue Shield (PPO plans only at this time*)

  • Cigna

  • Curative Health Care

  • Medicare**

  • Sana

  • United Healthcare** (Oscar/Optum/UMR)

*We are not currently accepting new BCBS HMO clients.

**We are not in-network with most Medicare replacement plans including Humana, Wellpoint, or United Healthcare Medicare.

If we’re out of network with your insurance company, our services will be private pay but we’re happy to provide a superbill so you can request reimbursement for any available out of network benefits. This option is not available to BCBS HMO policyholders.

Private Pay Rates:

90 Minute Initial Evaluation: $400

30 Minute Medication Management: $175

60 Minute Medication Management with Supportive Therapy: $225

We operate at a fixed rate and cannot offer sliding scale pricing.

New Patient Appointment Deposit Requirement:

To secure a new patient appointment, we require a $100 scheduling deposit. This deposit helps us reserve dedicated time for your initial visit and cut down on missed appointments.

  • This deposit will be held as a credit. Once the initial appointment is complete and any applicable insurance payments have been made, the deposit will be applied to the balance of your first appointment and any future services.

  • If your insurance covers our services at 100%, your deposit will be refunded.

  • If you miss or cancel your initial evaluation with less than 24 business hours' notice, the deposit is non-refundable. Arriving more than 15 minutes past your appointment start time may be considered a missed appointment.

  • If the practice needs to reschedule your appointment, your deposit will be transferred to the new appointment date or refunded upon request.

First Steps to Becoming a New Client:

  1. Please complete our prospective client contact form by clicking on the REQUEST NEW PATIENT APPOINTMENT button below.

  2. Select your preferred provider from the list.

  3. You must upload a photo of your current insurance card. Benefits are checked before we proceed with the onboarding process.

    **If you do not have insurance, you may upload a duplicate picture of your Texas Drivers License. Extra points given for a pet photo.

Please allow as many as 3 business days for us to verify your insurance benefits and reach out with next steps.

Once we have verified your insurance information, we will reach out by phone or email with your estimated out of pocket costs.

We will then forward our new patient paperwork and the $100 deposit invoice with a payment link.

We require the completion of all new patient paperwork and payment of the $100 deposit prior to scheduling your initial evaluation.

Incomplete forms or an unpaid deposit will delay the scheduling process.

**We may archive new patient requests if no progress has been made in 5 or more days.
