Below is some helpful information about our services, insurance accepted, and how to request a new patient appointment.

We are currently accepting new patients (ages 11 and older) for medication management and brief supportive therapy.

We Accept...
Blue Cross Blue Shield, United Healthcare (Oscar/Optum/UMR), Cigna, Curative Health Care, Humana, Medicare, Sana, and some Aetna plans.

If we’re out of network with your plan, we’re happy to provide a superbill so you can request reimbursement.
Private Pay Rates:
Initial Evaluation (90 minutes, in office): $400
Medication Management: $175 (30 minutes) or $225 (60 minutes)

Next Steps to Schedule the 90-minute initial evaluation:

  1. Please complete our prospective client contact form by clicking on the REQUEST NEW PATIENT APPOINTMENT button below.

  2. If you plan to use insurance, uploading a photo of your insurance card is required so we can verify your benefits.

  3. Select your preferred provider from the list. You can see who’s accepting new clients and read our provider bios here.

  4. We’ll send you the preliminary patient paperwork and an invoice for the new patient appointment deposit.

Deposit Policy

  • $100 deposit is required to schedule all new patient appointments. This deposit helps us reserve dedicated time for your initial visit and cut down on missed appointments.

  • This deposit will be applied to your first appointment balance.

  • If you miss or cancel with less than 24 business hours' notice, the deposit is non-refundable.

  • If the practice needs to reschedule your appointment, your deposit will be transferred to the new appointment date or refunded upon request.
